R3 Nomination Form is an advanced, production-ready WordPress nomination and multi-form management plugin. It enables site administrators to create and manage multiple customized nomination forms, embed them via shortcodes, configure dynamic visual appearance, toggle individual form sections, handle submissions with duplicate protection, and export submissions to CSV.

    • Name, Email, Phone, Company, Job Title, Relationship, Reason for Nomination, and Document Upload.
    • Optional and customizable Google reCAPTCHA v2 / v3 support.
    • Built-in duplicate submission detection.
  • Submission Management: View all nomination entries in the WordPress Admin, filter by status or date, view full entry details, and export to CSV.
  • Email Notifications: Send automated confirmation emails to both administrators and nominees upon form submission.
  • Secure & Standardized: Follows official WordPress coding standards, sanitization, nonces, and capability checks.

📦 Installation

  1. Download or clone this repository.
  2. Ensure the folder is named r3-nomination-form.
  3. Upload the folder to your WordPress /wp-content/plugins/ directory.
  4. Go to WordPress Admin > Plugins > Installed Plugins and click Activate under R3 Nomination Form.

🛠️ Usage

Embedding a Form

Paste the following shortcode into any page, post, or block:

[r3_nomination_form id="default"]

To embed a specific custom form instance:

[r3_nomination_form id="your-form-slug"]

Admin Settings

Navigate to R3 Nominations in your WordPress Admin menu to:

  • Forms: Create, edit, and configure form instances.
  • Submissions: View, filter, and export received nomination entries.
  • Settings: Configure notification emails, reCAPTCHA keys, and global options.
